Hospice Net, Inc.

The Hospice Net site features a wide range of informative articles aimed at making the end-of-life transition a gentle one for patients, caregivers, and surviving family members.  There is a special section devoted to children of varying age ranges, and how to help them understand the dying and grief process in a positive and healthy manner. 


Hospice Foundation of America


The HFA places a strong emphasis on education and grief support, offering a broad array of end-of-life care resources for professionals, patients, survivors and caregivers on their website.  There are numerous articles as well as many links to register for a large assortment of live programs and webinars.

Sponsored by the Hospice Foundation of America, HospiceDirectory.org provides a national database search tool to locate hospices by location and/or name, as well as links to additional information for both caregivers and patients.

National Hospice and Palliative Care Organization


The National Hospice and Palliative Care Organization (NHPCO) is the oldest and largest nonprofit membership organization representing hospice and palliative care programs and professionals in the United States. Their website contains a comprehensive amount of information and resources on all the elements of hospice and palliative care, with many helpful links to related organizations and relevant articles. 


The Mesothelioma Cancer Alliance

The Mesothelioma Cancer Alliance is dedicated to providing the most up to date and helpful information about mesothelioma cancer. Mesothelioma is a cancer caused by exposure to asbestos, and MCA is a top source for information regarding symptoms, treatment, and survivorship.

Aging with Dignity is a national non-profit organization with a mission to affirm and safeguard the human dignity of individuals as they age and to promote better care for those near the end of life. 


Caring Connections

A program of the National Hospice and Palliative Care Organization, the site provides free resources and information to help people make decisions about end-of-life care and services.  They offer a free multi-lingual HelpLine, a database of state-specific advance directives, and feature helpful articles on topics of interest for patients, caregivers, and survivors.


Physician Orders for Life-Sustaining Treatment

Physician Orders for Life-Sustaining Treatment helps to ensure that the treatment plans and wishes of seriously ill patients are honored by their medical providers. The website offers resources and information on how to identify existing programs or how to develop new ones, as well as offer tools and resources regarding the POLST Paradigm nationwide.


Put It in Writing

The American Hospital Association created this resource to encourage patients to explore their options regarding the preparation of an advance directive.  This site offers information about advance directives as well as links to worksheets, tool kits, and legal information.


National Advisory Center for Long Term Care Insurance

Website providing assistance with planning and selecting a Long Term Care insurance plan.  Features a free quote service, care cost calculator, and a direct help line.


Estatable

Estatable is a software service for estate administrators—i.e. Executors, Personal Representatives and/or Trustees named after a person’s death.  Estatable will help the estate administrator open an estate account, gather the decedent’s assets and organize the asset information.  Estate administrators who sign up while Estatable is still in beta will receive the service free of charge.

San Francisco Bay Area Network for End-of-Life Care
 is a networking and membership group for anyone involved in end-of-life care in the San Francisco Bay Area, with links to monthly meetings, newsletters and discussion groups.


The Coalition for Compassionate Care of California


The Coalition for Compassionate Care of California is a partnership of more than sixty regional and statewide organizations dedicated to the advancement of palliative medicine and end-of-life care in California. Their site contains information on research and public policy, guidelines for planning advanced care, and a link to download the California POLST (physician orders for life-sustaining treatment) form.


CancerPEN

The Stanford Cancer Palliation Education Network (CancerPEN) is a web-based palliative care national learning community. This is a resource for professional caregivers, that also links to the Journal of Palliative Medicine portal (http://palliativejournal.stanford.edu) as well as the Palliative Care Online curriculum a free, web-based study program.


Center to Advance Palliative Care (CAPC)

CAPC is a national organization run by the Icahn School of Medicine at Mount Sinai in New York City, dedicated to increasing the availability of quality palliative care services for those facing serious illness. It provides health care professionals with the tools, training, and technical assistance necessary to start and sustain successful palliative care programs in hospitals and other health care settings. 

Social Service Organizations  

Jewish Family and Children’s Services (JFCS)

Since 1850, JFCS has provided comprehensive social services to Bay Area residents of all ages and faiths. It helps solve problems and provides support to anyone facing transitions and personal crises, in order to strengthen the individual, the family, and the community.


Seniors At Home

Seniors At Home is the senior services division of the Jewish Family and Children’s Services organization.  Seniors At Home houses a unique community-based Palliative and End-of-Life Care Program, and has created a process for the transition involving caring for the dying at home.


Final Passages

Final Passages is a model project offering education for personal and legal rights concerning home or family-directed funerals and final disposition (burial and cremation). Its intention is to reintroduce the concept of funerals in the home as a part of family life and as a way to deinstitutionalize death.

The AARP website provides an informal setting with a variety of articles and personal interest stories related to grief and loss.


George Mark Children’s House

George Mark Children’s House serves families with children who have a serious medical illness and who are seeking family-centered medical care that emphasizes quality of life in a compassionate, supportive atmosphere.


Hospice Foundation of America


The HFA provides articles and videos on grief, and offers assistance with finding a support group.


Institute on Aging

The Mission of Institute on Aging is to enhance the quality of life for adults as they age by enabling them to maintain their health, well-being, independence, and participation in the community.

Josie’s Place is a San Francisco Bay Area program specifically aimed towards helping children and their families facing the loss of a family member or friend.
