THREE STEPS TO ADMISSION:
- Review our General Criteria for Admission and then call the Intake Staff at 415-913-7682. They will be able to answer any questions you have and can inform you about availability of rooms.
- Download our application using the link at the bottom of this page. You will be asked to fill out several documents for our internal use to establish that the referral is appropriate.
- If the criteria are met, you will be asked to provide additional information, including mandatory documents from physicians and health care providers. These include a documentation of Pulmonary Tuberculosis status, requiring a chest x-ray or a skin test to determine that there is not active TB, and you should be aware that the skin test takes three days for the results to be read.
If all the steps are completed, the new resident will be admitted – either immediately or if the house is full, when a bed becomes available.
WAIT LIST
If a bed is unavailable, then wait-time for a bed varies. Potential residents will be placed on a waiting list and will be informed when a bed becomes available. Please feel free to call and check on the status of your referral at any time. The main number at the Guest House is 415-913-7682.

Zen Hospice Project reserves the right not to accept any resident who does not meet the admission criteria. We also reserve the right to make exceptions on a case-by-case basis regarding residents that might fall within or outside the basic admissions parameters.
COSTS
Every resident of the Guest House is required to be registered with an outside hospice agency and is responsible for payments to that agency. Every resident is also responsible for the custodial care (room & board) they receive at the Guest House which is based on a sliding scale and evaluation by our intake staff. Charges for room and board are billed directly to the resident. Medicare, Medi-Cal, and most private insurances will not reimburse for these charges.
Thus, residents are responsible for:
- Room and board payable to Zen Hospice Project, which includes lodging, all meals, 24-hour nursing care, volunteer support, and other Basic Services.
- Hospice care payable to the resident's own hospice agency, which includes support from a clinical hospice team who are trained in symptom management and end-of-life care.
